All full-time employees are eligible for life insurance coverage through Unum Insurance Company, providing $50,000 Basic Life coverage and $50,000 AD&D (Accidental Death & Dismemberment). Full-time employees become eligible on the first day of the month after the completion of 30 days of employment. All full-time employees will also have an opportunity to purchase additional life insurance for themselves and their dependents.

Election Information:
Under SA’s group plan, employees can purchase supplemental life insurance in increments of $10,000, to a maximum of 5 times their annual salary, or $500,000. Life insurance can be purchased for spouses in $5,000 increments, up to 100% of the employee’s coverage. For children, life insurance can be purchased in increments of $2,000 to a $10,000 maximum.
Guaranteed Issue:
Another benefit of SA’s plan is that there is a ‘guarantee issue’ amount of $140,000. That means that you do not need to have any medical underwriting to qualify for at least $140,000 worth of supplemental life insurance. These premiums are based on age and other determining factors. If you elect coverage over the guaranteed issue amount, then you will need to submit responses to a medical questionnaire provided by Unum.
Your premium will increase every 5 years.
